If you did not print or save your certificate at the time of completing the test and need to request a copy, please contact us at the email below with the following details:
- Your name
- Your profession
- Your NHS board
- Date you completed the test

Please note: we cannot issue duplicate certificates for certificates appearing with “Unknown” in the fields. If you are experiencing this problem please consult the Frequently Asked Questions page for a solution.
